Registration To register, you must complete and return the Camp Registration form. Enclose a check for $200.00, made out to Connecticut Elite Field Hockey Camp, LLC. Applications are accepted on a first-come basis. The remaining balance must be paid by May 1st, 2010. However, we will accept applications AFTER May 1st, if they are accompanied by a check for the full camp fee. In addition, a completed Youth Camp Examination Record form must also be returned before the beginning of camp. This form is good for three years. Each camper is required to bring a check to registration for $100 as a room key deposit. When the camper returns her room key at the end of camp, she will receive her room key deposit. Please make checks payable to: CEFHC, LLC We appreciate your cooperation, as the University of Connecticut charges the camp $100 for each lost room key. Each camper will be given a lanyard for the purpose of keeping her room key safe for the entire camp session.
